get that far, turn right on to Shadeland and pass through
the intersection of 21st and Shadeland. Then, follow this

To save time at
your first appointment as a new patient, you can download
the forms (pdf format, 18 pages) that we would hand you
at the front desk. These require Adobe Acrobat Reader. You
can then print them out, complete them, and bring them in.
We would like for you to complete everything, but
you are free to decide what applies to you after
the first nine pages. Everything after that is quick and
easy to complete; it's not as hard as it looks!
Yes! In fact, the integrative medicine approach
at Health Synergies is an ideal complement to whatever treatment
your existing doctor(s) may be providing. We will provide
him or her with on-going progress reports.
